Over the past decade our group has developed many forms, guidelines and information sheets in an attempt to keep the providers current in Geriatric medicine, long-term care and governmental regulation in these settings. Our patient population consists primarily of the frailest elderly and our assessments and guidelines are modified to reflect this frailty. In addition, tools have been developed to assist nursing home staff to provide more comprehensive assessments of changes in residents’ conditions. These tools have given the nursing staff more autonomy and have improved the quality of communication between the group’s providers and the nursing staff.
